While most issues between client and clinician can be addressed directly, some situations could require escalation. In compliance with the State of Texas, we provide information below to assist your pursuit of a report, if you should find yourself in such a situation.

Request Records

Texas Records Access and House Bill 4224 Notice

In accordance with Texas Health and Safety Code Section 181.105 and House Bill 4224, clients have the right to request access to or copies of their health care records. Requests must be submitted in writing and may be subject to reasonable processing time and allowable fees under Texas law. Records will be provided within the time frames required by state and federal regulations unless access is restricted by law.

 

If you are a legal guardian or court appointed representative, documentation verifying your authority must be attached to this authorization.

We are required to retain your records for 7 years, or 5 years after a former child client has turned 18.

Good Faith Estimate

Under the "No Surprises Act," as of January 1, 2022 medical facilities are required to provide a "Good Faith Estimate" to patients not planning to use insurance. This Good Faith Estimate shows the costs of items and services that are reasonably expected for your health care needs for an item or service. Please let your provider know if you plan to seek services as a self-pay client, and this can be provided to you. For more information go to: https://www.cms.gov/nosurprises.
